How to refer.
Use the referral form or call the referral line on 020 7093 4710. The form asks for the client postcode area only. We take the clinical picture from you by phone.
After discharge.
The reality of returning home after a stay in hospital can be difficult, but with help from Soma, we can assist you during your recovery period to help you get back on your feet.
